Orlando Luz and Rafael Matos defeated Constantin Frantzen and Robin Haase in the final, 6–4, 3–6, [10–7] to win the men's doubles tennis title at the 2026 Cincinnati Open. It was their second consecutive ATP Masters 1000 title, having previously won Montreal, thus becoming the first pair in men's doubles to win both the Canadian and Cincinnati Open titles in the same year since Pierre-Hugues Herbert and Nicolas Mahut in 2017, and the fifth pair overall to do so in the Open Era.

Nikola Mektić and Rajeev Ram were the defending champions, but did not compete together this year. Mektić partnered Austin Krajicek, but lost in the second round to Marcel Granollers and Horacio Zeballos. Ram partnered Joe Salisbury, but lost in the second round to Frantzen and Haase.
